CLIENT AGREEMENT

THIS AGREEMENT (the "Agreement") is dated at time of purchase with the payee, (the "Client") and the Jayde Scrymgeour Glowing and Thriving ABN: 79973301613

 

By making this purchase you agree that you:

  • Have read and agreed to all of the client policies listed
  • Are 100% responsible for your own decisions and results
  • Authorise Jayde Scrymgeour Glowing and Thriving to charge payments to the account details provided upon first payment as in line with the fee schedule / payment plan stated on this page.

Services

  • Subject to the terms of this agreement, Jayde Scrymgeour Glowing and Thriving must supply the Services to the Client in consideration of the Client paying the Fee to Jayde Scrymgeour Glowing and Thriving
  • Jayde Scrymgeour Glowing and Thriving will commence the provision of the Services on the Commencement Date, and continue to supply it until this agreement is terminated pursuant to these terms.

The Client must:

  • Provide assistance to Jayde Scrymgeour Glowing and Thriving as reasonably required so that Jayde Scrymgeour Glowing and Thriving  may competently perform its duties under this agreement; and
  • Jayde Scrymgeour Glowing and Thriving  will attempt to obtain the Goals of the Client. The Client acknowledges that Jayde Scrymgeour Glowing and Thriving  makes no guarantee that the Goals can be achieved.
  • Complete all necessary modules, tasks or homework and attend all scheduled programs to the best of their ability in the endeavor to reach the goals set between Jayde Scrymgeour Glowing and Thriving & The Client.

Fees

This contract secures your place in the VIP Coaching or program directly with Jayde Scrymgeour Glowing and Thriving. By signing this contract, The Client is agreeing to recognise that payments are non-refundable and by taking a place in this container, you are under contract to complete the entire duration of your payment plan. Therefore you cannot stop payments anytime. You are under contract to complete the entirety of the payment plan. The Client is responsible for any fees incurred by a Debt Collection agency.

  • You agree to have funds available in your account for payment processing before the scheduled charge date. Failed or late payments will incur a 5% additional fee.
  • This program has no cancellation policy. You are under contract to finish the entire payment plan even if you decide to not finish the program.
  • Transfers are not available to any other course/programs of Jayde Scrymgeour Glowing and Thriving 
  • The fee payable by the Client is inclusive of GST if the clients primary location is Australia.



Upon signing this form I agree to the following:

  1. The purpose of a nutrition consultant and holistic health coach is to improve the overall health and well-being of the body through nutritional education and holistic lifestyle support. A nutrition consultant and holistic health coach does not diagnose diseases, disorders or conditions, and simply provides recommendations and suggestions to improve overall health based off the information provided.
  2. As part of the holistic service I may be asked to provide information concerning my diet and nutritional habits, physical habits, medical history, moods, energy levels and lifestyle. This information will be kept in confidence and will not be disclosed to any other person, without my consent, except as required by applicable law.
  3. If there is a suspicion of disease, disorder or condition, I will be informed of this suspicion; however, I acknowledge this is not a diagnosis and will be referred to another professional who holds additional qualifications. 
  4. Glowing and Thriving is in no way liable for my health or safety.
  5. I understand that Glowing and Thriving reserves the right to determine which cases fall outside their scope of practice, in which event an appropriate referral will be recommended.
  6. I maintain full responsibility for all decisions as they relate to my health and wellbeing, and the health and wellbeing of my baby. I will use your own judgement and discernment before implementing any suggestions provided, and if in doubt will perform my own research and discuss with my healthcare provider.
  7. I understand that the ultimate responsibility for my health and my baby's health is my own responsibility.
  8. I have read the above waiver of liability, and fully understand its contents and voluntarily agree to the terms and conditions stated.
